กรณีเจ็บป่วย

1. แบบฟอร์ม ตม.7 (คนต่างด้าวต้องมายื่นคำร้องด้วยตนเอง)
2. สำเนาหนังสือเดินทาง, วีซ่า, ตราประทับอนุญาตครั้งสุดท้าย
3. หนังสือรับรองจากโรงพยาบาล โดยให้ปรากฏคำวินิจฉัยของแพทย์ดังนี้

- เจ็บป่วยเป็นโรค หรืออะไร

- เป็นอุปสรรคต่อการเดินทางไกลหรือไม่ อย่างไร

- ต้องรักษาตัว หรือไม่สามารถเดินทางไกล นานเท่าใดแต่เมื่อใด เป็นต้นไป

กรณีดูแลผู้ป่วย

1. แบบฟอร์ม ตม.7 (คนต่างด้าวต้องมายื่นคำร้องด้วยตนเอง)
2. สำเนาหนังสือเดินทาง, วีซ่า, ตราประทับอนุญาตครั้งสุดท้าย
3. หนังสือรับรองจากโรงพยาบาลที่ทำการตรวจรักษาผู้ป่วย โดยระบุความสัมพันธ์กับผู้ป่วย หรือหนังสือรับรอง ความสัมพันธ์กับผู้ป่วยจากสถานทูต หรือสถานกงสุล

กรณีเพื่อเยี่ยมญาติหรือกลับภูมิลำเนาเดิมของผู้เคยมีสัญชาติไทยหรือของผู้ที่มีบิดาหรือมารดา เป็นบุคคลสัญชาติไทยหรือเคยมีสัญชาติไทย

1. แบบฟอร์ม ตม.7 (คนต่างด้าวต้องมายื่นคำร้องด้วยตนเอง)
2. สำเนาหนังสือเดินทาง, วีซ่า, ตราประทับอนุญาตครั้งสุดท้าย
3. หลักฐานแสดงว่าเดิมเคยมีสัญชาติไทย หรือแสดงว่าบิดา หรือมารดามีสัญชาติไทย หรือเคยมีสัญชาติไทย

กรณีเพื่อเยี่ยมคู่สมรส หรือเยี่ยมบุตรซึ่งมีสัญชาติไทย

1. แบบฟอร์ม ตม.7 (คนต่างด้าวต้องมายื่นคำร้องด้วยตนเอง)
2. สำเนาหนังสือเดินทาง, วีซ่า, ตราประทับอนุญาตครั้งสุดท้าย
3. หลักฐานแสดงความสัมพันธ์

(กรณีคู่สมรสต้องมีความสัมพันธ์กันทั้งทางนิตินัย และพฤตินัย)

3.1. สำเนาทะเบียนบ้านของคู่สมรส หรือบุตร

3.2. สำเนาบัตรประชาชนของบุคคลผู้มีสัญชาติไทย

3.3. สำเนาทะเบียนสมรส หรือสำเนาสูติบัตรของบุตร

กรณีเพื่อการดำเนินคดี หรือดำเนินกระบวนพิจารณาอันเกี่ยวกับคดี

1. แบบฟอร์ม ตม.7 (คนต่างด้าวต้องมายื่นคำร้องด้วยตนเอง)
2. สำเนาหนังสือเดินทาง, วีซ่า, ตราประทับอนุญาตครั้งสุดท้าย
3. หลักฐานยืนยันว่าเป็นผู้เกี่ยวข้องกับการดำเนินคดี โดยเป็นผู้กล่าวหา ผู้เสียหาย ผู้ต้องหา โจทก์จำเลย หรือพยาน

กรณีปฏิบัติหน้าที่หรือภารกิจให้แก่ส่วนราชการ หรือรัฐวิสาหกิจ หรือหน่วยงานอื่นของรัฐ หรือสถานฑูต หรือสถานกงสุล หรือองค์การระหว่างประเทศ

1. แบบฟอร์ม ตม.7 (คนต่างด้าวต้องมายื่นคำร้องด้วยตนเอง)
2. สำเนาหนังสือเดินทาง, วีซ่า, ตราประทับอนุญาตครั้งสุดท้าย
3. หนังสือรับรองหรือร้องขอ จากส่วนราชการระดับกรม หรือเทียบเท่า หรือหัวหน้า
4. รัฐวิสาหกิจ หรือหัวหน้าหน่วยงานอื่นของรัฐ หรือจากส่วนราชการตำรวจระดับกองบังคับการหรือเทียบเท่าขึ้นไป หรือจากส่วนราชการทหารระดับผู้บังคับหน่วยในสังกัดกระทรวงกลาโหม กองบัญชาการกองทัพไทย กองทัพบก กองทัพเรือ กองทัพอากาศ ซึ่งมีชั้นยศพลตรี พลเรือตรี พลอากาศตรีขึ้นไป ในส่วนที่เกี่ยวข้อง หรือจากองค์การระหว่างประเทศ

กรณีมีเหตุจำเป็นโดยมีสถานฑูต หรือสถานกงสุล รับรองและร้องขอ

2. สำเนาหนังสือเดินทาง, วีซ่า, ตราประทับอนุญาตครั้งสุดท้าย
3. หนังสือรับรอง หรือร้องขอ จากสถานฑูต หรือสถานกงสุล

Tem­po­rary Visit Visa

1. Expats need to pass a stamped visit visa.
2. Non-​nationals or those who are reg­u­lated by the follow-​up com­mit­tee of immi­gra­tion offi­cers of Immi­gra­tion Bureau Offi­cer. (In cases of being Sri Lankan, Indian, Pak­istani, Bangladeshi, Iran­ian, Nepali, Togolese, Niger­ian and Ugan­dan, one will be per­mit­ted to stay in the King­dom not more than 7 days.)

Required doc­u­ments

1. Appli­ca­tion form TM. 7. (An expat needs to apply oneself).
2. Copy of pass­port, visa and lat­est stamp.
3. Photo 4×6 cm. size(not more than 6 months).
4. Fees 1,900 Baht.

Spe­cific case doc­u­ments

In cases of ill­ness

• Appli­ca­tion form TM. 7 (An expat needs to apply oneself).
• Copy of pass­port, visa, lat­est stamp.
• Cer­tifi­cate from hos­pi­tal with med­ical diag­noses fol­low­ing these:

- Ill­ness because of dis­ease or want.

- Is ill­ness an obsta­cle for a remote travel or not.

- Be treated or not, how long he or she is avail­able for a remote travel, since when.

In cases patient treat­ment

1. Appli­ca­tion form TM. 7 (An expat needs to apply oneself).
2. Copy of pass­port, visa and lat­est stamp.
3. Cer­tifi­cate from the hos­pi­tal that treated the patient, men­tion­ing rela­tion­ship with the patient, or cer­tifi­cate of rela­tion­ship with the patient from the embassy or consular.

In cases of rel­a­tives visit or domi­cile return of those who had Thai nation­al­ity or those whose father or mother has or had Thai nation­al­ity.

3. Cer­tifi­cate demon­strat­ing that one had Thai nation­al­ity or demon­strat­ing that father or mother had Thai nationality.

In cases of spouse visit or chil­dren visit with Thai nation­al­ity

1. Appli­ca­tion form TM. 7 (An expat needs to apply oneself).
2. Copy of pass­port, visa and lat­est stamp.
3. Cer­tifi­cate of rela­tion­ship.

(in cases of spouse visit, needed de jure and de facto rela­tion­ship)

3.1. Copy of house­hold reg­is­tra­tion of spouse or chil­dren.

3.2. Copy of Iden­tity card of Thai nation­als.

3.3. Copy of cer­tifi­cate of mar­riage or cer­tifi­cate of birth.

In cases of tri­als or trial process

1. Appli­ca­tion form TM. 7 (An expat needs to apply oneself).
2. Copy of pass­port, visa and lat­est stamp.
3. Cer­tifi­cate of being on trial process accuser, accused, plain­tiff, defen­dant or witness.

In cases of being on duty for offi­cial an orga­ni­za­tion, state enter­prise, other gov­ern­men­tal office, embassy, con­sular or inter­na­tional orga­ni­za­tion.

3. Cer­tifi­cate or request from department-​level offi­cial sec­tor or equiv­a­lent or head of state enter­prise, head of other gov­ern­men­tal office, police department-​level orga­ni­za­tion or equiv­a­lent up, chief of mil­i­tary sec­tor in Min­istry of Defence, Royal Thai Armed Force Head­quar­ters, Royal Thai Army, Royal Thai Navy, Royal Thai Air force with major gen­eral, rear admi­ral, air vice mar­shal up in related sec­tor or inter­na­tional organization.

In nec­es­sary Cases cer­tifi­cated or requested by the embassy or con­sular

1. Appli­ca­tion form TM. 7 (An expat needs to apply oneself).
2. Copy of pass­port, visa and lat­est stamp.
3. Cer­tifi­cate or request from the embassy of consular.
